OK, so she seems interesting.

Ask yourself the following questions:

Are you interesting?
With all the guys in her circle of friends, WHY should she be interested in you, a quasi-stranger?
Is your behavior a stalking behavior?

Go out there, go somewhere where people socialize: I bet you can find at least 10 girls who are just as pretty and just as interesting as this girl, go approach them. It's OK, it's easy, /r/seduction has talked ad nauseum on the technique on how to do this.

Probably depends on the relationship with the mutual friend, and how well your friend knows them. It might just be an old high school acquaintance and you look like some kinda FB stalking creep. But my all means plow ahead if you think you have a chance.

If she accepts your friend request, here's a fun thing to do. Most of the time when girls post a pic on FB, everyone fawns over it telling her how cute she looks. Do the opposite. Not like say she's ugly, but comment on the background of the picture, if it's one side of her face say you like the other side, that kind thing.
