Well, you could have your attorney write her a letter, then take her to court with a motion to compel. It will likely cost you $3-5000.

But seriously, do you really care? 2 years later, are you really keeping up with her so much that her having the same name is worth all that trouble?

I don't think a clause like that is enforceable in court. Other than a money judgement, anything else in the decree is mostly worthless

Mine still has my last name too. I was a terrible abusive husband and she cheated on me, but let's keep the last name. It doesn't matter anymore - she can do what she wants

Mine cheated on me and wants to keep it. The only thing I can think of is to keep it consistent with the kids.

You have to realize that in divorce court the only thing they actually care about is where the money is coming from and where it's going to go to and the division of assets and debts. Everything else that they write into their divorce decrees is just fluff to make it look like they care.

Nothing. There’s nothing you can do. She can keep your last name, or she can go back to her maiden name. She can get remarried and keep your last name if she wants.
