Never got the "why". In my own experience, most girls will say yes and give you her number even if they are uninterested, because they prefer not answering on an online chat than saying "no" face to face and avoiding the awkward moment. I partially answer this in point 4
Be direct, do it in under a minute of talk. My opener is always "Hey, name" smiling and shaking hands. As if you were a greeter at the GAP. She will probably say her name afterwards, like "Hi, Juliette" (shake hands). And then repeat her name a couple of times in the conversation. It's an easy way to remember her name and it will make her feel good. One of my lines is usually "Juliette, you looked bored, what's up?" and then continue from there. I used that line in the gym and in a coffeeshop with a lot of success.
Wait a day and text her "It's name from the coffeeshop, what's up?" "what are you up to?"
Be as if you were having an important meeting in 10 minutes and have to leave. After a minute or two of chit-chat, one of my lines is usually "Hey, I'm on my way out, do you have a whatsapp?". If she asks why, then be straight up "so we can set something up to have a proper talk one of these days"

This is what has worked for me. So take it for what it is.

I give her that look that says "Duh, seriously?". And if she still doesn't get it, "Because it's going to be hard to call you without it"
I usually set it up that I need their info to tell them details about the plans I/we already made. "I'll give you a call tomorrow." and then I'll pull out my phone. If they don't rattle off their number I'll say "How do you spell your name?" and then they usually get the hint.

If you ask, and she says "why," it means you already fucked up somehow. Ok, most of your questions boil down to the same thing which is you're not being confident and assertive. Girls know exactly why you're asking for her number.

I do this: Get her phone (most girls have their phone in their hands 24/7, but you can just tell her to give it to you like "hey give me your phone"). Either add yourself on facebook, instagram, whatever, or put in your phone number and call yourself from her phone which gives you her number. Optional: if you added on facebook or whatsapp or whatever, take a selfie with her and send it as the first text in the convo so she will remember who you are the next day.

DO NOT: Ask for anything. Don't ask for phone numbers, ask for dates, etc. Don't say "want to go out with me sometime?", say "Hey, I'm going to X tomorrow afternoon, come meet me there. give me your phone i'll give you my whatsapp".
