I think it fades over time if they really stop watching porn. However, we still live in a pornified society. Even if he actively tries to avoid being exposed to any kind of porn, he will still see objectified women and girls in advertisement (yes, girls, because it's common for modeling agencies to hire girls of 12 or 13 as first time models) and it will impact how he views women.

Well I used to browse an anti masturbation site where men would attempt to quit porn. They frequently said that quitting porn made women become "human" to them again. Also that they began to see the beauty in "real" women again and stopped being creepy. So there's that. I suspect once the chemicals in their brains return back to normal, they improve.

First one depends on the guy. "spank bank" can either refer to masturbating to porn scenes, or to own sexual experiences of the past, or to the fantasy of a specific woman/specific sexual acts doing with that woman.
