It's just the bitch-shield they put up. The other day I had to get around this girl so I put my hand on her shoulder (from behind) and said "Scuse me" to gently get her out of my way. She turned around and gave me this fuckin look like I had just killed her cat. I laughed at her and said something like "Just trying to get around you," and then she was all smiles.

Reactions that happen quickly like that are usually automatic. They're not really a reflection on you, but try not to open from behind so much. If you open from behind, you generally want to assume attraction (obviously) and talk like you fuckin know she's gonna like what you have to say. I often barge into sets just super high-energy, and usually the girls won't give an automatic negative response.

Edit: I should also mention, the reason that one instance sticks in my mind is because I usually don't get a reaction as horrifying as the one this girl gave me. She literally looked at me like I was a monster, but instead of letting it get to me, I just deflected it and moved on. She said "Sorry" with a cute little smile immediately after giving me a death stare. Two things to take from that: One, girls tend to put up an automatic bitch-shield (there's a lot of reasons for it, but don't let it fool you into thinking you did anything wrong). And Two, girls (and guys) don't like getting approached from behind if they don't know you're coming (I mean, usually they're cool with it, but sometimes it'll fuck you up). So if you do approach from behind, be high-energy and friendly so they let their guards down.

While I don't bark out replies at people, I am the proud owner of a perma-bitch face when I'm not thinking. So it's a borderline sneer before someone comes up to me and I remember to soften my features. It's not always your fault! Some of us just have grouchy neutral expressions.

Make sure you're not confusing honest lack of communication skills on your part with a bitch-shield/shit test. If you walk up to a woman and start talking, by the time she realizes you are there, talking to her, she just missed half of what you said, hence a very normal "What?" on her part. This is why you should touch their arm briefly as you say "excuse me" or "hi guys", and once you have their attention THEN begin your opener. You'll find a lot of those "shit tests" you're seeing will stop happening.

Well the problem is you should make eye contact & stand in front of them. That way they know you are talking to them & they are focused on you.
