Being creepy is a lack of social calibration. Because there's something "off", it scares people.
Approaching when it's dark. Smiling at a funeral. Stalking a girl and not approaching. Inability to take a clear rejection. Saying morbid jokes to people you just met.
You fix this by socializing more and calibrating towards building comfort.

If you get called a creep or creepy once of twice in your life I don't think it's a big deal. If you get called that fairly regularly I'd say you need to make changes.

Believe it or not women don't know what a creep is. Ask a few girls what's a creep. They won't have a definition, but they will have examples of what they think it is. You can walk up to a girl, say hi and she might think you're a creep. You can walk up to a different girl, say hi and she'll think you're normal guy being friendly. We all have different perceptions of what a "creep" is.
Neediness and self worth are a mindset thing. I recommend reading Models for that. Low self-worth means you need to raise your confidence. This has been said before in this sub million time, if you want to raise your confidence 1. work out/exercise 2. improve your style, hygiene 3. practice working on your body language and posture.
You should never see yourself as a weirdo. If you feel misunderstood that's normal. From reading your post I would try working on communication and try to stop caring how people see you. If you believe your creep, then you might act like one. If you believe you're an awesome guy with extraordinary interests, fun hobbies ad person people want to be around then you'll start seeing you're as an awesome guy.

There's a good quote in Mark Manson's 'Models' that goes something like.
If you're going to pursue women, there will be times when you will be called a creep. Taking risks means you might make some people uncomfortable.
How you avoid it, is responding with emotional intelligence if you are obviously making someone uncomfortable. Apologize or shrug it off with a comment and change the subject.

You must not have been paying attention man because posts about being a "creeper" are up on here all the time. In my view, being a creeper is self fulfilling. If you're constantly worrying about being a creeper, you're going to come across as creepy in everything you do. If you just chill out and be comfortable and confident with yourself, that will come across in everything you do.
Creepy guys are the ones constantly worrying about being creepy.

"Creepy" is just a cop out word for "unattractive" that people use when they want to feel less shallow about disliking someone. There's not really a way to avoid it other than guiding them to a different derogatory word. Just call them uncreative if they say you're creepy.

It doesn't take an intricate understanding of both parties involved when the situation amounts to,
Boy: "Hi."
Girl: glares at boy until he gets uncomfortable and leaves "Geez, that guy was such a creep."
Pretty much every time I've heard the word "creep" used unironically in a social setting this is a summary of what actually happened. I'm not including media portrayals of it since those are fictional events.

I'm confused. Do you honestly think there exist no other possible scenarios in which people are called creepy? A stranger staring at you in public transport? A superior asking you inappropriate questions? An acquaintance always managing to brush up against you 'accidently'? A socially awkward person not knowing when to quit an approach? Why ignore those possiblities?

I'm saying that I've never seen it used in that context outside of staged interactions. The closest I've seen is a superior getting called gross after making a suggestive joke with his subordinates. Every time I've heard "creepy" get used outside of a joke it's been an "I don't like that person" knee-jerk reaction.
For all the situations you're describing there's a different word that gets used like weirdo(public tranport), gross(superior/aquantence), predatory (also superior/aquantence), and the last one doesn't really get an accusatory word since that reaction can be summarized as "take a hint". Any interaction can be seen as creepy if you're looking for a cop out. When people have an actual problem with behavior, and not the people involved, they don't use the word "creepy" they use the situationally appropriate language.
I have never seen any exceptions to this behavior. This is why when someone says "you're creepy" they're saying that they dislike you, not your behavior. It's a cop out word that gets used when you want to degrade someone under the pretense that they're unacceptable to society instead of just being unacceptable to you.

Ok, you certainly don't have to take my word for it, and I doubt you will, but please consider this: Confirmation Bias exists, the Baader Meinhof phenomenon exists and even if you were the most neutral person in the world, your personal experience is not representative of everyone's. To me it sounds like you personally associate the term with that particular scenario and you feel a tad attacked by it, which is fine. But fact is, the term, in its definition and usage, is not as specific as you have depicted it, others use it differently and I don't understand why you would deny any association but your own. It's a weird hill to die on.

"Creepiness" is simply attention (usually of a sexual nature) from an undesirable suitor.
You can solve this by either being desirable, denying women attention or simply both.
Practice ignoring people. Don't even look in their direction. When you do, make the interaction brief before focusing on something else.

The appearance of confidence. If you're nervous they'll pick up on it and start speculating why you're nervous and that'll lead to conclusions that you are not being sincere, worried about being found out and creepy.
Women want to know where they stand, that's one of the main appeals of what people describe as being "alpha". Most of that is BS pseudo-science. What's important is that someone who is like that is being sincere in their communication of intentions - even if they are joking and being insincere in what they are actually saying. The woman knows where she stands and is flattered by the attention. The guy is taking charge of the situation and she can enjoy the ride, doesn't have to worry about having to steer things herself.
