Honestly it's all about the girl you're with and the signs she's giving off. If she's looking at your face and your lips a lot you are good to go. If she tries to avoid you when you get close to her don't do it. Be smart and if you mis interpret her signs and she says no or stops you, it just shows her how much confidence you have. But if she's showing a lot of negative signs don't do it. As a general rule the second or third date is the latest I like to leave it.

In order to kiss her, you have to create environments in which a kiss might happen. You're talking about a relationship, so I'll answer in that context:

Pre-reqs: Conversation has flowed, you're decently comfortable together, you're receiving indicators of interest even as simple as "the look". The official relationship is not mandatory before the kiss.

Forget all this if you haven't been really good with kino. Touch the elbow, touch the thigh in a seated situation, put your arm around her (even just hooking your arm around her and the person on your other side in a group environment), or my favorite: clear strands of hair out of their face. It's cute.
get to a decently secluded place! This one is important, and probably one of the biggest problems you've had. A garden path, the theater prop storage room, a playground at night, a bench on a cool night under the stars — all appropriate. Doesn't have to be a private place, just one where there aren't a ton of people. At dinner in a dining room, at school during the day — probably won't cut it.
A touch of adventure helps a lot, but is not required. Discover the prop room and try out different hats and props, playing a pirate, then a villain, than a cowboy. Hop the fence to the playground and try out the swings. I live in a semi-arid desert, so it's a good time to look for a kiss whenever it rains.
Make sure there's an intimacy to the moment. Physical proximity is huge here, as going to a kiss from three feet away can be really awkward. Capture her by swinging your Zorro cape around her, stepping in close so you're together in the semi-darkness. Sitting, put your arm around her and look into her eyes with a quizzical look.
The subject of conversation is unimportant. You can even interrupt her, saying "stop" before you kiss her. It's helpful to have had sexual-themed conversation before, but it's not necessary.
As soon as you touch foreheads, the kiss is just about inevitable. Physical proximity must have already happened.
Ask her before you kiss her. Not mandatory, but it's sweet, polite, gentlemanly. If you do it while you're touching foreheads, your answer is going to be yes. If some of the above steps haven't happened, you've got a high chance of her saying no or maybe.
If there's any chance that one of you is inexperienced or out of practice, touch her cheek and guide her a little bit; it'll anchor both of you and decrease miss %.
Kiss her. Short, sweet, lips together at first. Feel free to repeat and escalate if that went well.

I've been on a lot of first dates recently and here's what's been working great for me. I park the car a little ways away from the place we're going. Like a block or two away. Then when were ready to leave I go for the hand hold, you can tell a lot about how comfortable a girl is by how she holds your hand. If you let go and she hangs on, is she squeezing your hand etc. (Also holding hands is not that big of a move and is therefore easy to do). Then as we're walking to the car and talking I say "hang on a second" and turn her with the hand I'm holding to face me. Then I make some serious eye contact and then the kiss. Works every time for me so far. I like to stop the make out before her and then turn her back around and give her a little push to the car. Also try to find any excuse to touch them during the date. Rings are great for this: "what's the story behind this ring?"

Edit: Furthermore, I think it's a good idea to ALWAYS go for the kiss on the first date unless it's painfully obvious she doesn't want to. She already knows if she wants you to kiss her at that point anyway. I just assume they want me to unless they make it obvious they don't. The only thing worse than being rejected from a kiss is wondering if you could have kissed her when you didn't go for it.

I had a girl over and we hit it off pretty well. I could tell she liked me but there werent really any signs like locking eyes for a while or something so I said fuck it "can i just kiss you already" worked out well

It's always been easiest for me to get her looking in your eyes, try to stop her while you're walking and compliment her eyes. Better to take a chance at a kiss than wait too long. At the VERY least, there must be a good night kiss.

Don't ask, that just ruins the moments and shows a lack of confidence. The way that works best for me, is to be joking around with them in one way or another, and say a good line in their ear (if it's loud, or if you're in a movie), and then just position yourself in a close eye contact position, and if they're even remotely interested it will just happen.

Don't over think it, try to maintain some eye contact with her. If it's being held for a while try to lean in closer. If she's still holding eye contact with you then you have a green light up to this point. Go for it then.

I found the best way to know is looking at the eyes. If her eyes aren't moving, they get bigger and she stares right into your eyes without flinching then I slowly move my face closer to her face. Then talk about random shit that keeps her talking while tucking the hair from her face behind her ear (in prep for the kiss, she'll know what's up), then grab her neck with your hand that's been moving her hair, and lean in and go for the kiss. I find it works every time. Granted they'll move away sometimes, but only after you get 1-2 true kisses. Here you can recalibrate to either ease off, or get more aggressive depending on her vibe. But hey now you're definitely not friend zoned.

If you don't know when the right time is, any time after about, say, 2 hours of interaction is ripe for the smooching.

Just get her into a secluded spot and go for it. Tell her, "Hey I wanna show you something," and pull her into a dark alley or where ever and make your move.

Seriously, just do it.

Set a timer for 2 hours on your first date and when it goes off (put it on vibrate so you don't mess with the mood), make your move. Don't worry about IOIs or any of that shit.

If you're in a club/bar and the alcohol and tunes are flowing, you can shorten that timer to 10 minutes if the girl is DTF.
