You are not wrong to not want your GF to have contact with her ex if that is something really important to you.

Personally, I couldn't care less if a girl is in contact with her ex. In fact, I would find it odd if they were not.

Making rules like that makes you look insecure.

She broke the rule, you gave her a final warning, and now she is almost but not quite stepping over the line, testing you out.

The rule has revealed an insecurity of yours and now she is manipulating you with it (probably unconsciously).

What you actually needed was a boundary not a rule. If you have a boundary (which is a rule for yourself, not for her) that you will split up with her if she is in contact with her ex, you have to follow through. But you haven't. It leaves you in an unenviable situation, where creating silly rules causes trouble down the line.
