What’s the worst thing that could happen?
For men, it’s getting kicked out for being a creep or potential sexual assault allegations (I’m talking absolute worst).
For women, the man will let you down nicely (or harshly but the odds on that one really are slim to none).
There’s no inherent risk in a woman approaching a man in the gym as long as she’s respectful and brief about it. If he’s uncomfortable, and he lets you know, it literally can’t be interpreted as sexual assault or harassment.
So the only real risk is your pride getting crushed.
Just be short, sweet, to the point. And if he rejects you, tell him to take it as a compliment, have a nice workout, and then you both go on your merry way.

Literally no man has ever gone up to the front desk and said "this woman is making me uncomfortable with her attention and advances… could you have something done about this so that I can work out in peace?"

That is such wrong advise .. If he doesn’t ask you out after you initiated conversation doesn’t mean he’s not interested..most guys are like me for example: unless a girl straight up tells me that they want to go out, or if they are obviously flirting like calling me handsome and touching me , i will not understand any hints.. if a girl came up to me at the gym asking me to spot her then I wouldn’t think she’s hitting on me , I would just think she just needs a spot ... the best thing to do to go hit on that guy is to just ask him straight up if he wants to go out.. if he’s interested he will say yes ... all men wish girls were more simple and just ask us instead of throwing hints

Every time I've come across a girl in a shared social setting like a gym or a school and we've got the mutual "eye-rape" thing going....Every single time, she was interested and I was being a big pussy by not taking the hint.
One of them looked like the archetypal out-of-your-league bartender and when I did finally work up the nerve to talk to her, she gave me her number like she had been waiting months for me to ask for it.
And if she had approached me first and broken the ice, something is wrong with you if you don't at least shoot your shot.
It's not fair to expect girls to do all the heavy lifting of getting into her pants. You're not that awesome, and you're expecting too much out of her. It will happen from to time, as it's happened to me, but in almost all of those cases, she was horny AF and I only happened to be the guy in her crosshairs.

You don't have to necessarily walk right up to him and ask him on a date, but at least try to strike up an actual conversation with him to signal your interest, instead of just asking him to do some favour for you and hoping that he takes that as an opportunity to strike up a conversation himself.

Guys are supposed to lead. If he's too much of a pussy to say something after being approached, then he's not worth her time, simple as that.
The trick is the vibe and sending the message that you're only using the request for a spot as a means to break the ice. That's why my advice to OP was to come in a little flirty, introduce yourself afterwards and then walk away to get him to chase.
To be fair, this is something where girls often outsmart themselves. They come up with the right move, but are so afraid of putting themselves out there, even when they already are and want to, that they come in with the wrong energy/mindset and leave the guy thinking "was she flirting with me, or just being friendly?"

It's tough to say if this is direct enough advice, cause the gym has a serious stigma around approaching girls. I would contest that "if he doesn't hop on that opportunity he's not worth it" for that reason alone. But its up to you
It all comes down to what vibe she brings to it. If she comes in flirty and bubbly, then he's being a pussy/idiot.
If she treats him like a piece of gym equipment and doesn't use the opportunity to flirt at all, then of course the guy is going to be confused and unsure what her intent was.
What women think is subtle and what men think is subtle are two completely different things. And some of the best practitioners of girl game that I've seen knew how to make their intent clear without beating people over the head with it or embarrassing themselves. They start learning this in junior high when guys are still just getting out of the "girls are icky" phase.
UPDATE I haven't seen him the last few times I've been at the gym. But I'm prepared for when I do!

If you can catch his eye and smile that should do the trick. Option 2 should option 1 fail,,,,, is ask him to spot you or help you with something like a clamp, or pin or your shoelace... Option 3, should option 1 and 2 fail, would be to approach and ask him if he'd like to meet up at your local lunch spot for coffee or snacks. He will be flattered, but if he is an intense gym person, get him as he is finishing up. He will be much more receptive.
