Never with a one-word answer, never negative, always appropriate, always with a reason.

I'm good, I just had a great dinner... (really? what did you cook...)
I'm alright, work was crazy, any ideas on how I can relax? (yeah, sometimes I ...)
I am fantastic, I just had an amazing shower (what made it so good...)

It's a banal question that you can spice up to show some personality, so don't leave it at "I'm doing great".

...Well, it was amazing because the soap slipped out of my hand and I caught it in mid-air, all that childhood T-ball finally came in handy...

Depends how they seem to be doing. If they look depressed try to be sympathetic while being honest, 'Not bad, but could be better, how about you?' If they look happy then you can chalk it up more, 'Doing pretty good, certainly nothing to complain about.'

I try to avoid over the top things such as super, fantastic, amazing. To me it just seems like people who respond like that are either crazy, or faking it (of course this is just my personal experience). I usually use words like good, alright, bored, great, excited. Then I continue on to give them a short (!) explanation why, and use that to provoke a further conversation.

For example today at work I had a customer ask me 'How are you doing?' My response was "I'm doing good. I'm getting off soon, so its always a good time." This response has sometimes led to (female) customers asking me what I'm doing after work, and even inviting me to come out and drink/bowl with them (I work at a bar/bowling alley).

Tonality makes a big difference when answering this question to. There is a huge difference between "I'm doing good.." and "I'm doing good!" Make sure your response and tonality go together or it will be obvious that your faking it.

I love it!!!! I never know how to answer this question...with actions??? "just working hard" ...with feelings???? "feeling a little tired" or just plain "fine".

You don't always have to answer the question directly- especially if you already know the person a little bit and they are just asking out of formality.

If you have asked a girl how she is doing, and she responds then asks how you are doing, you can just ignore her question and respond with an answer that is some sort of flirty follow-up to her situation. This requires a little bit of calibration in order to reduce the chance of you coming off as a jackass.

Sometimes however, ignoring the question and connecting the conversation immediately to how she is/ what she is doing can up the energy level of the conversation, and subtly frame the conversation in your favor (cocky-funny frame).

Quick, witty anecdote. You gotta think fast. Practice on cashiers, they usually ask how you're doing. Have something quick to say. dav0s got the right idea. Examples I would have answered yesterday:

Good, I saw some sort of film production set on the way over here. Did you hear anything about that? (wait for the answer) because I was told to stop walking, and when I asked what was up, he was sparse on details.
Incredible, I can't wait to do my laundry when I get home, everything smells like hamster's cage. (dhv: you don't live at home)
Good, I can't wait to go biking, tell me you're AT LEAST going be out in time to enjoy the sun. (dhv, pseudo-conversation)

I hate this question but I try to turn it around on people.

Me: How are you?

Clerk: Good, you?

Me: Wait, really.

Clerk: Um... yeah

Me: Naw, that's not good enough. I don't believe you. Everyone says "good" because they're supposed to. No one gives a real answer. How are you actually doing right now?

Clerk: Well, a bit tired, but otherwise I'm actually pretty good.

Me: And a bit tired of the wise ass on the other side of the counter.

This thing used to kill me when I was young. I still hate it. I used to take that for a serious question and tried to produce a serious answer like "....mhmmm... wait.. I got to think about that.." (Yeah I'm such a nerd =D) Today I would say it SO doesn't matter what you answer. Just nod, say "mmhh", whatever.. people don't want to know, people often don't realize they are asking a question (!!) - it's like "hi" to them. Best to just throw it back at them: "Hey how ya doing?" "Yeah! What's up with you?" ( <- See no answer, mommy =D) Unless of course it's someone who's really interested in how you are doing - but than they will ask it later in the conversation not at the beginning.
