"We analyzed all deaths among pregnant women and postpartum women (≤42 days after delivery) in the United States from 2018 through 2023 with the use of the National Vital Statistics System. International Classification of Diseases, 10th Revision (ICD-10) codes were used to identify cause of death and pregnancy status; a universally used checkbox also identified pregnancy.

During the 6-year period, 7901 deaths occurred. The leading cause of maternal death was unintentional drug overdose (1152 deaths; 5.2 deaths per 100,000 live births). Violence (defined as homicide or suicide by any means) was the second most frequent cause (866 deaths; 3.9 deaths per 100,000 live births). Homicides accounted for 593 (68%) of all violent deaths and suicides for 273 (32%). Firearms were involved in 77% of homicides and in 39% of suicides. Cardiovascular causes ranked third (Figure 1 and Fig. S1). In all, violence and overdose accounted for 2018 deaths; the next four most common causes of death (cardiovascular causes, hypertension, infection, and hemorrhage) together accounted for 2141 deaths."

As only 2/3rds of "violence" is "homicide" (including by people other than intimate partners), it actually ranks lower than cardiovascular or infections with COVID included.

It has been pointed out three years ago here on PPD that homicide being "the" leading cause of death among pregnant women is mathematically impossible. Most sources claiming or suggesting the opposite are paywalled; among those that aren't, at least one made a rudimentary math mistake (included homicide within a year after pregnancy and did not adjust for time frame).
