The point of saying hurtful things is to emotionally or mentally harm someone.

Most men are sensitive about their penis size and ability to have sex.

As such, if a woman is trying to hurt a mans feelings, she will bring up a personal or general insecurity of men.

That doesn't mean she sees that man sexually. It means that she knows that man is emotionally and mentally hurt about specific topic related to his sex and sexuality.

I had an ex who said I had a small penis after I started dating her friend... She had never even touched it other than through my pants once. But when her friend that I started to date said that she said that but that “I don’t believe her” it really bothered me for a long time and it totally worked on me as I lost confidence and we stopped dating. I had another hang up (I have small hands for a guy) and it kind of reinforced that insecurity (a guys hand size correlating to penis size) at the time too. Thank God I got through that though. Women can be evil. The only reason I dated her friend was because I felt like she was a much better match for me and the first girl was not going to work.

That’s the point of the insult. Men routinely tell us they get validation from sexual attention from women. The insult is just another way of saying you’re not a sexual object, because women know men want to be one. It’s an insult. They don’t necessarily always make sense.
