I never believe these stories when I read them. For a man to make a mistake like this, he would have to be saying the muscle tone of the anal sphincter, a non receptive one at that, is identical to the softer folds of a sexually aroused vagina.

I've had this happen to me and be a legit accident but I definitely think my experience is an extreme outlier. We were teenagers going at it in a bathroom standing up.
