One thing I rarely see get covered in the whole NiceGuy rigmarole is the details of escalation and how it can create a dicotomy where the same behavior produces wildly varying reactions

Consider the common advice on TRP for making hookups happen. The man is advised to start making physical contact with the girl, to move things further physically given the opportunity, and to try and set up the circumstances for sex to happen. And plenty of girls will be just fine with this, but some won't. They won't want to be touched, won't want to be pushed towards sex right away. The more demure man might decide to ask about these things first or not to push them, but will often find himself going nowhere because of this.

So, is the guy who presses forward confidently an asshole for not asking first? Is the demure guy nice or a loser for not doing so? Is what separates a confident guy and an asshole not their actions, but merely that one is attractive and one is not?
