The love languages is a great tool (but often badly misunderstood by most) for facilitating these sort of conversations.

And finally once these things are discussed how many of you are actually making a real plan to make sure your partners needs are met?

E.g. My wife and I have different love languages when it comes to gifting. I don't really care about gifts but they are important to my wife in terms of feeling loved and appreciated. We had a very deep discussion on this year's ago, and we discussed exactly what all her expectations are for birthdays, anniversaries. Valentines, Christmas, random times etc. As a result of this I have more than a dozen recurring notifications in my phone to make sure I fulfill all her needs to the best of my ability. A notification popped up on my phone this morning "Order Valentines flowers, book restaurant, gift?" So I've already ordered the flowers this morning, I have a card written with a sweet message, and I've booked a restaurant. I also reset the reminder for a gift until tomorrow when I'll have time to go out and buy one.
