If the "mileage ring" is the only additional condition, I wouldn't mind at all; I'm a huge electronics nerd.

Now, let's see what we can do in terms of mileage calculation. We basically need a distance sensor that can work in abundant bodily fluids without interfering much with the process, and an integrator. I'd go with hermetically sealed RFID coils mounted on stickers attached to her opening and his base. But if you're super into capacity sensors, maybe two stickers with capacity sensors along the penis' length would do the trick even better. They're hard to strap electrionically though. Resistive sensor will inevitably be sensitive to bodily fluids' composition and can interfere with lube if it's used, so the only option we're left with are sonic and optical sensors. I don't see how they can be used accurately.

Accelerometers were suggested, but they'll count such movements as changing positions as mileage.

Plus, we're left with the philosophical question - do rotational moves count as "mileage"? Entrance teasing? Sliding? How do we measure blowjobs? Handjobs?

These are not simple questions.

Update: We can use optical sensor similar to the one used in laser computer mice - a low-res camera, a correlation calculator, and a laser diode. Currently, they eixst as an on-chip solution that works with minimum strapping.

First thing that popped in my mind is an accelerometer. They're precise enough to provide the answer for mileage.
You can probably do something small enough to glue to a guy's hip with arduino and a accelerometer shield, but if you design a custom circuitry I suppose you can have it built fitting a cock ring without spending too much.
That would still require you to ask your partner to wear a ring or some paraphernalia though.

Although hard to do, I'm pretty sure you can design a tape small enough for you to just stick it in the vagina and leave it. Maybe investigate the variation of capacitance to make the measurement continuous, I don't know. I caught myself thinking of this one, but I managed to stop myself from thinking enough to make some advancements worthy of note. I totally got trolled though.
