Lose facial fat. It will enhance your facial features.

Get a clear skin. Use facewash and scrub. There are medicines for acne. Moisture and scrub your lips.

Honestly man I used to think this way. One thing that changed me for the better was just developing a more positive attitude on life, including my self image. You obviously take care of yourself along with going to the gym. As corny as it sounds sometimes you just gotta look in the mirror and start “feeling yourself”. Not literally, but like gas yourself up a little. The sooner you get out of the mindset of “I’m ugly, or I look like sloth from goonies” the sooner you’ll have some more confidence in yourself. if you have actually maximized your looks potential (muscular physique, low bf%, good haircut etc.) then focus on other areas where you aren't 10/10 yet. be it finances, social skills, education, you name it. there's always room for improvement.

You have to up your game too if you want to attract them. Get a GQ or go to a fashionable store and have a hot girl friend go with you. Get clothes that fit perfect.

Get a really cool haircut and just find that edge to your style and grooming. You’ll be surprised how many ordinary looking people (men and women) can look elevated with the right style.

I have exactly 2 friends who are on the very ugly side but always surprised me for how they managed to get beautiful girlfriends (even more so than more handsome guys I know), this mostly due to them being very charismatic and witty.
Traveling has also made me realize that there are certain countries in the world where the ratio of beautiful women to ugly men is so high (i.e. slavic countries like Poland) that you very often see these types of pairings. In these countries I was surprised for seeing model-grade women doing totally unexpected jobs like cleaner, trash/waste collectors, cashier etc.
So aside from working on yourself, start looking into the possibility of traveling or moving to a different country.
