What she said was a massive low blow, 100% weaponized to hurt him where she knew it would sting the most. That's a huge red flag about how she fights. The real question is if she said it just to be cruel in the heat of the moment or if that's a deeper resentment she holds.

They need to talk about it when they're calm. If she can't give a sincere apology and understand the damage, then he's got a big problem.

I think some injuries cannot be healed. I don’t know how someone can use that type of venom against another in a relationship. I’ve been in your friend’s shoes in a different way. Ultimately, she sounds awful and at the very least therapy would be a must if he wants to try and salvage anything or even for him alone and move on from her.

He needs to get to the underlying issue of why she’s being nasty. She has lost respect and admiration for him for some reason. He needs to work out why and fix it or move on. Don’t worry about the comment itself or his size. Women usually don’t care about that, if he becomes obsessed with that it will destroy him and the marriage. Women need to respect and look up to their man to be in love, right now she doesn’t so figure out why and fix it or end it. This clearly violates the "honor" part of most wedding vows I have ever heard. It weaponizing male sexual jealousy. Sexual jealousy is primal in males because we are never sure our children are ours. Over an evolutionary time scale, men who were sexually jealous and kept and eye on their mate passed on their genes. Those who were not, were less likely too. Women know how to exploit this.

Another one is when you are dating a woman and she is constantly saying "a guy I dated, I guy I dated" about a bunch of different guys. Of course that comes across as "a guy I had sex with" and its a way of rubbing her body count in your face.

My ex-wife when we were first dating make an offhand comment that the last guy she was with couldn't get it up and she thought it was her. I did not need to hear that. I got angry with her about that comment and she dumped me before I coaxed her back. I should have responded "yeah the last woman I was in a relationship really knew how to take it from behind.," which was actually true.

I don't know if this one comment is a reason to divorce, but the marriage has serious problems. Contempt is a leading indicator of divorce, I read once. You can't un-ring a bell.

Sadly this is just the way some women fight. If I had to guess I would say he probably expressed this insecurity to her at some point and she dug it up and turned it against him when she wanted to hurt him.

That's the reason I would say he should get a divorce. Not because she said his dick was small, but because she wanted to cause him pain and she was willing to to sacrifice intimacy and trust in order to achieve that ends. She cares more about "winning" the fight than about him as a human being.

That's not the kind of thing that changes about a person, she's shown her true colors. If your friend can't accept that his relationship is always going to be like this in one way or another, then he should leave. If he thinks a marriage isn't much different than a knife fight and feels like he can hang with that... I guess stay.
