I scheduled a time to get in and talk about the big snip, but I was told that my wife would need to join me or sign a consent form.

The doctors office said that this consent form is for legal reasons, and takes them out of any issues that might come from the snip.

Has anyone else ran into this before?

UPDATE : I have called and spoke to my doctors office, asking for a referral to a different urologist and explained to my doctor the consent from i was told my wife would have to sign. The doctor listened, and I got the impression this is not the first time this issue has come to her attention, and told me no problem they would get one to me shortly.
I then asked if they would refer women to an OB-GYN if the women were required to have their husbands fill out a consent form for their birth control. She said more then likely they would not, and that it was a very good point.

When I called the urologist office to cancel, the woman there asked why I was canceling the appointment. I explained my opinion on their consent from and she replied with, 'That is fine'. Again getting the impression I am not the first person to have an issue with this.

At this point I am waiting to hear from the local University hospital for a consult.
