Sounds like they performed the study without proper oversight and approval. Also, I'd be interested to find out exactly what prohibits this study, I doubt it is anything specific to circumcision. There are a lot of processes that need to be followed when doing research with any living thing and especially humans. If they had actually followed those processes and their research actually did violate ethical regulations I doubt it would have actually gotten to the point where they had data to release.
Now, all of my experience with research is in the US with US organizations, so the above might not actually apply to Canadian research.
First, the text is not clear on whether or not it is not allowed due to law or due to the particular ethical code that the hospital follows for research.
Second, my issue is if they were infact performing research without oversight on human subjects, not that they were studying the affects of circumcision or if the study is allowed or not. The use of human subjects, particularly ones that are not able to consent, without oversight is a massive ethics violation in and of itself, regardless of intent.
Consent and oversight are a pretty big deal in research with human subjects for a couple reasons. The first reason is that it is very easy to mistakenly do harm to a subject, proper oversight can help prevent this and the subject should know the risks of the experiment. On top of that, historically there have been many experiments done on humans without any regard to the safety of the persons experimented on. For these reasons pretty much all experiments on humans require some kind of oversight. The fact that these researchers went around this process is enough make the study itself not viable to publish.
While these researchers did not go this far here are a couple famous examples to explain why it is such a big deal in the research world:
It is human experimentation because the subject was a human, regardless of whether or not the experimentation involves potential physical harm or not. This is pretty much one of the first things anyone involved with research learns about. Causing physical harm is not the only ethical concern during experiments, so it is not the only criteria for "human experimentation".
Anyway, the whole point of oversight is that the person performing the research is not the one who is allowed to determine if the experiment is ethical. While in this case the experiment might not have actually been unethical, the fact that they did not follow the proper processes means that they can't prove it was actually performed in an ethical manner and undermines any data produced. If they cut corners (completely ignored) on what is considered by many to be one of the most important parts of research, where else might they have done a poor job?
Strange, when I click that link it takes me to the wrong page. So I can't see that page for some reason. Are you sure it's talking about Paul D. Tinari, and not Paul Tinari? Because the first guy is the author of this study and the second guy is a 3D printing engineer who has nothing to do with it.
Both have Director, Pacific Institute of Advanced Studies listed, and both went to Queen's University, but they graduated different years and their photos are totally different.
Actually, that's one of the problems I had: There's two people with very similar names. One is a 3D printing engineer. The other is the "author" of this paper. One of the sources published in this thread links to an article with a picture of the wrong guy on it. Try searching Paul Tinari on LinkedIn and you should get 3 results. One is the guy you're talking about. The other is the author of this paper.
I don't. I had hoped that someone else had one. I'd hoped that, since he published as much as he did, that he'd actually taken a stand and defied the wishes of his censors. And if he'd gone this far, then maybe the full article existed out there somewhere on the web.
OK, before anyone gets worked up, I think MGM is abhorrent and we should all have a right to foreskins. I am however reserving judgement on the regulations until I have read them myself. That said, this study caught my eye for looking somewhat suspect, and it just got worse and worse the more I looked.
No control. This invalidates the result entirely. Without a child put in the exact same position minus only the snip itself, there's nothing to say that the trauma wasn't caused by being restrained, being out in a noisy, claustrophobic MRI machine, being separated from family, being manhandled by a doctor, some other unknown affects, or any combination thereof.
n = 1. One is not an acceptable sample size. For all we know, this kid is an outlier, and experiences more MRI-visible trauma than 99.9% of the population or vice versa. Sure, it's unlikely but the point remains that any deviation from the population mean shown in the scans is now our best guess at the whole effect.
Either one would be a death knell for the scientific credibility of their conclusions, and would get them laughed out of any reputable journal. I would assume that Dinari's focus on the single patient is indicative of there only being one, but as I was writing this up, I realised something - as the results are supposedly destroyed, Dinari is under no obligation to tell us about his whole sample.
If he got to the point where he was presenting results, it seems incredibly suspect that he would go in with only the data he mentions here due to the aforementioned reasons. It could very well be the case that there were more children that had experienced a less dramatic or persistent effect that Dinari leaves out - after all he isn't obliged to show his working. This is of course all conjecture, but this opinion piece based off only a single subject strikes me as incredibly odd.
For fun, here are some more easy targets:
Follow up testing happened at one day, one week, and one month. I really shouldn't have to point out why that doesn't constitute evidence of a "permanent change".
Despite the prevalence of non-anaesthetised circumcisions, this result can only be used to draw conclusions about those particular circumcisions. Not including that in the conclusion is a notable flaw. For all we know, the trauma disappears when you add an analgesic into the mix, the effect of which on pain is currently being tested in . . . Canada?
I'm getting bored here, but the final thing I want to bring up is that the consen given here is a grayscale rat's-nest:
The operator of the MRI machine in the hospital was a friend of mine, and he agreed to allow us to use the machine for research after normal operational hours. We also found a nurse who was under intense pressure by her husband to have her newborn son circumcised, and she was willing to have her son to be the subject of the study. Her goal was to provide scientific information that would eventually be used to ban male infant circumcision. Since no permission of the ethics committee was required to perform any routine male infant circumcision, we did not feel it was necessary to seek any permission to carry out this study.
There are a few other things I left out, like whether the analysis was blind, and the general methodological vagueness, but suffice to say, I would not be content using this as evidence in any scenario.
That still doesn't address the lack of a control. I know it seems pedantic, but you've got to control for factors like restraining the baby's head and body, placing them in the confined (and possibly noisy) MRI Scanner with a stranger, who fiddles with their bits for the same amount of time using a blunt knife and bell.
Basically do everything but snip, to exclude the possibility of any other factors causing the trauma, because a control is not merely a non-treatment group. It's an identical-but-for-the-treatment group.
Yea they say snipped people have their brain permanently altered in this study but from my experience on this sub anytime the topic of circumcision comes up the uncut folks lose their minds. This sub is lost when it comes to the topic
Yeah I'm willing to bet this would happen to anyone who had any sort of painful surgery without anesthetic...i mean come on, what sort of study was this and who the fuck would do that to anyone much less a child?
If you are going to do a study like this it probably should be done exactly as they would in any clinic, as far as i know if you told the doc to use an obsidian blade and no anesthetic youd likely be thrown out on your ass and a call to cps would likely be made. Beyond that your movement would likely get more support if you lightened up on this issue you guys are nuts on this topic

One of the sources published in this thread links to an article with a picture of the wrong guy on it. Try searching Paul Tinari on LinkedIn and you should get 3 results. OK, before anyone gets worked up, I think MGM is abhorrent and we should all have a right to foreskins. I am however reserving judgement on the regulations until I have read them myself. That said, this study caught my eye for looking somewhat suspect, and it just got worse and worse the more I looked.
No control. This invalidates the result entirely. Without a child put in the exact same position minus only the snip itself, there's nothing to say that the trauma wasn't caused by being restrained, being out in a noisy, claustrophobic MRI machine, being separated from family, being manhandled by a doctor, some other unknown affects, or any combination thereof.
n = 1. One is not an acceptable sample size. For all we know, this kid is an outlier, and experiences more MRI-visible trauma than 99.9% of the population or vice versa. Sure, it's unlikely but the point remains that any deviation from the population mean shown in the scans is now our best guess at the whole effect.
Either one would be a death knell for the scientific credibility of their conclusions, and would get them laughed out of any reputable journal. I would assume that Dinari's focus on the single patient is indicative of there only being one, but as I was writing this up, I realised something - as the results are supposedly destroyed, Dinari is under no obligation to tell us about his whole sample.
If he got to the point where he was presenting results, it seems incredibly suspect that he would go in with only the data he mentions here due to the aforementioned reasons. It could very well be the case that there were more children that had experienced a less dramatic or persistent effect that Dinari leaves out - after all he isn't obliged to show his working. This is of course all conjecture, but this opinion piece based off only a single subject strikes me as incredibly odd.
For fun, here are some more easy targets:
Follow up testing happened at one day, one week, and one month. I really shouldn't have to point out why that doesn't constitute evidence of a "permanent change".
Despite the prevalence of non-anaesthetised circumcisions, this result can only be used to draw conclusions about those particular circumcisions. Not including that in the conclusion is a notable flaw. For all we know, the trauma disappears when you add an analgesic into the mix, the effect of which on pain is currently being tested in . . . Canada?
I'm getting bored here, but the final thing I want to bring up is that the consen given here is a grayscale rat's-nest:
There are a few other things I left out, like whether the analysis was blind, and the general methodological vagueness, but suffice to say, I would not be content using this as evidence in any scenario.

That still doesn't address the lack of a control. I know it seems pedantic, but you've got to control for factors like restraining the baby's head and body, placing them in the confined (and possibly noisy) MRI Scanner with a stranger, who fiddles with their bits for the same amount of time using a blunt knife and bell.
Basically do everything but snip, to exclude the possibility of any other factors causing the trauma, because a control is not merely a non-treatment group. It's an identical-but-for-the-treatment group.

Yeah I'm willing to bet this would happen to anyone who had any sort of painful surgery without anesthetic...i mean come on, what sort of study was this and who the fuck would do that to anyone much less a child?
If you are going to do a study like this it probably should be done exactly as they would in any clinic, as far as i know if you told the doc to use an obsidian blade and no anesthetic youd likely be thrown out on your ass and a call to cps would likely be made. Beyond that your movement would likely get more support if you lightened up on this issue you guys are nuts on this topic
