It's not gender affirming care at all. Men go bald, baldness doesn't take away from their expression of gender identity. You could probably make a better case for conditions that make women lose hair.
Whether gender affirming care should be a part of social heathcare is highly controversial. Not becouse people are whatever-phobic but becouse most cosmetic - or whatever judged as "non-essential" - procedures are not covered by most social healthcare systems. Example: reconstructing the abdominal muscles after childbirth is covered by healthcare but if you also don't want your stomach to look like a dog's ballsack, that counts as a non-essential cosmetic surgery therefore it isn't. Same thing with dental care and pretty much everything else. Healthcare doesn't cover the best solution, it covers the cheapest solution. So many people think - justifiably - that gender affirming care has no place in the conversation as long as hundreds of "non-essential" procedures that are more generally needed are not covered by social healthcare.

The WHO includes AGA in its classifications of diseases, specifically within the chapter for "Diseases of the skin and subcutaneous tissue" in the International Classification of Diseases (ICD). It is generally considered a common medical condition with a significant psychosocial impact.

Where I live, women who lose their hair are given state support for buying a wig, whereas men are not. They simply make a distinction between the reason for baldness, knowing full well that one affects women and the other men. Yet they also state that in the rare case that its androgenic alopecia causing female baldness, an exception can be made. Which shows that their rules are gender discriminatory, yet they try to hide it because its against their own laws.

Men can already stop male pattern baldness by using oral finasteride and oral minoxidal in early stage baldness when thinning is just starting.

Can also microneedle 1.5mm kinda hurts but helps a lot too.

Well, not for men & trans masculine people at least. But I could see the opposite being true actually (for a trans woman whom this would be gender affirming to/for in the traditional sense)! Not saying that’s bad/invalid or anything either, if it’s just cosmetic so being done for men’s self esteem/well-being regardless, btw. Baldness does not make men feel like their appearance does not match their gender. Baldness is primarily associated with the male gender so while it may cause discomfort, it does not cause one to feel like they look like a different gender.

I don't think either should be subsidized, tbh. And I say that as a woman with very advanced hair loss that I hate and mentally struggle with. I mean I'd say I'm somewhere between Norwood 4 and 5. Minoxidil and stabilizing my hormone levels has helped a little (and I keep taking the Minoxidil hoping for a more delayed effect) but I still have very high temples, and what hair is in between them is still struggling to cover my scalp. I wear a scarf or headband all the time to hide this disastrous hairline. It's a challenge to make my hair look even remotely feminine, but I do what I can with what I've got, in cutting, dyeing, styling.

So I can actually understand that men who go through balding likely have a similar battle with their hair, and trying to find way to still look attractive. But thing is I probably don't look nearly as bad as I imagine, and the same is probably true for most balding men too. Some men even look more attractive with high temples or no hair. And sure some don't, but isn't that just the genetic lottery in general?

But as much as I know losing one's hair sucks, and as much as I can empathize with men going through the same thing, the haur loss itself still a vanity issue and not an actual health concern. Poor mental health can't be actually solved with cosmetic surgery. There are tons of people (yes, mostly men, but also a few women) who went through with hair transplant and they're still not happy about how they look. Take Joe Roegan for example. He went through a hair transplant, but he still decided to embrace the bald look, and now is unhappy with the big scar on the back of his head that the transplant left him with. And then there are cases where hair transplant isn't even an option, like if you don't have enough, or good quality enough, hair to use as a donor site.

Would I love to get a free hair transplant? Absolutely, it would make me jump and dance from joy. It would be the end of my hair misery. Or at least maybe cause me to have less bad hair issues. But that doesn't mean I think it should be my right to get a free hair transplant. Because my balding isn't causing me any actual functional issue that can't be solved by wearing a frigging hat. Because hair in itself doesn't have a lot of actual function to begin with. It may to some small extent protect the head from cold temperatures and harmful UV rays, but like I alluded to, a hat can do that too. Not feeling attractive is not a health issue.

The same can be said for gender dysphoria. It's a mental health issue. You get therapy for mental health issues, you get surgery for physical health issues. Mixing that up, there's zero actual proof that works.

But yes, we should take mental health serious too, and I'd have no issue with therapy being subsidized to anyone who needs it. It's a part of health care. I also have nothing against cosmetic surgery, like I may try to save up for a hair transplant or similar procedure at some point. I just don't think cosmetic surgery falls into "health care" unless it's causing issues with actual physical function, or a patient has been wronged or botched by a doctor in the past, or by injury, birth defect, or disease. Like for example a messed up nose that actually needs rhinoplasty to improve breathing function, or reconstructive plastic surgery to fix botched or lost body parts. And sure, we could say that baldness is technically a loss of bidy part, but it's not due to birth defect, injury, disease or hack doctor. Unless it is, but then if so that's different.

Point is there's nothing wrong with getting a hair transplant or any kinda cosmetic procedure if it's what you want and something you can benefit from. But to claim it should just be handed to you for free just because trans people somehow managed to find a loophole in the medical ethics for "treating" their mental health issues, I think is kinda pathetic. You wanna look better, then you pay for it. Or just try some Minoxidil/Rogaine and/or what's that DHT-inhibitor called now again. Unattractiveness is not a disease.

The "gender affirming care" stuff for plastic surgery is stupid. It's always cosmetic plastic surgery, on a trans person on a cis person.

The state should therefore fund hair transplants for all men who have advanced past the Norwood 2 stage.

But this is exactly why - even if accept your argument - it shouldn't be covered by public dollars. Hair transplants are an art. When do it, what stage what age, what technique to use, where to place the hairs and how many. All that impacts cost.

The fact men who are dealing with male pattern baldness will likely need to take Finasteride or a like drug the rest of heir lives to maintain the results weirdly makes it like an organ transplant. It brings a level of medical ethnics into it - is the patient responsible enough to take care of this "gift"?

With that said, covering Finasteride for hair less could be argued for.

So first of all, I’m not against insurance/socialized healthcare covering cosmetic procedures in some cases. If a licensed mental health professional determines that someone is experiencing significant distress from a certain feature or body part and is not merely addicted to cosmetic surgery, I think it should be covered. Trans people have to go through this process to get their procedures covered, and it makes sense others would have to as well.

However, hair transplants are not gender-affirming care. Men tend to go bald. Going bald does not cause a man to resemble a woman, it simply causes him to look older. Many can fix this by just shaving their head and growing a beard, it doesn’t work on everyone but tends to be an attractive look.

Now, things like breast implants for flat-chested women and breast reduction for men with gynecomastia would be much better examples of procedures that could be considered gender-affirming care for cis people. But ultimately I don’t think that should be the determining factor… it should be the therapist’s decision. Because you have some folks who do not see their bodies accurately and could claim getting massive boobs or a lantern jaw as “gender-affirming care,” deforming themselves on the taxpayer’s dime. It needs to a) be essential for someone’s mental health, and b) be relatively safe and not disfiguring.

You’re stretching “gender-affirming care” to mean any cosmetic trait tied to sex or age. If hair counts, then breast implants, jaw surgery, height surgery, acne treatment, and weight loss all count too.

You’re also abusing the word dysphoria. Distress about hair loss is real, but that doesn’t make it clinical gender dysphoria. If genetic insecurity equals dysphoria, the state owes surgery for almost anything.

“Lowers quality of life” isn’t a medical standard. Almost everything lowers quality of life to some degree. Baldness is common, usually non-disabling, and compatible with normal life.

The Norwood 2 cutoff is arbitrary and cosmetic. Once you apply this logic consistently, it collapses into subsidizing cosmetic surgery.

My first reaction to this was that we should normalize baldness—and I’m a woman who shaves my head because of hair loss, and I just don’t want another problem to manage medically, so I’m probably a bit biased here—but it looks like there are studies that show that male-pattern baldness is a significant source of distress for many men and that treatment generally improves their quality of life.
